Practice Policies

New patients: Any new patients that do not show for their first initial appointment without notifying us--CCPM&R will attempt to re-schedule once. If the prospective patient does not re-schedule, CCPM&R will contact the referring physician/provider. If the prospective patient is a self-referral, then CCPM&R will request a non-refundable deposit of $50.00 to re-schedule a second time, and any further attempts to re-schedule. This amount will be applied to the initial visit charge when the prospective patient attends the first appointment. This policy allows CCPM&R to see as many patients in a timely manner and provide the best possible care in covering overhead expenses.

Appointments: Please arrive 5-10 minutes before your scheduled appointment time if the New Patient paperwork has been completed. If unable to complete and submit the New Patient paperwork prior to the first appointment, then we ask you arrive 15-20 minutes early to your scheduled appointment. Arriving late may cause us to re-schedule the appointment. If you need to re-schedule or cancel your appointment, we ask you to please notify us, preferably 24 hours before the appointment time to possibly schedule another patient on our waiting list. We recognize that things can happen, and plans need to be altered. Several missed appointments may cause CCPM&R to discharge you from the practice. We appreciate your understanding in this regard to help as many people as possible in the context of the work schedule.

Medication refills: Please contact your pharmacy when needing refills. Your pharmacy will notify CCPM&R through an electronic prescription system.
