New Patient Information
After initiating contact with CCPM&R through phone or email, a new patient will receive a packet by email, fax, or mail. In the packet, there will be some forms to complete for health history information (about 5+ pages), the release of information forms for medical records from other physicians and providers, and consents for treatment (especially for minors).
The patient will be seen at one of the various satellite clinics that CCPM&R has established around the Quad Cities and Eastern Iowa, which will be determined with patient input.
The initial visit at CCPM&R will be longer and more costly in general than subsequent visits.
